You don't need to scrub into surgery to be great at sterile processing — but understanding what actually happens in the OR makes you dramatically better at your job. Knowing why a tray is assembled a certain way, or why a specific instrument matters, turns memorization into real understanding.

Why SPD Technicians Should Understand Surgical Procedures

Sterile processing exists to serve the OR. Every tray you assemble, every instrument you inspect, and every sterilization cycle you run is in direct support of a surgical procedure happening (or about to happen) on a patient. Technicians who understand the purpose behind instrumentation catch errors that a technician working from memorization alone might miss — a missing dissector, a mismatched retractor, an instrument in the wrong position for how it will actually be used.

The Three Phases of a Surgical Procedure

  1. Pre-operative phase — the patient is prepped, positioned, and the surgical site is marked and prepped. This is when the OR team confirms the instrument trays and equipment needed are present and sterile.
  2. Intra-operative phase — the actual procedure. Instruments move between the surgeon, scrub tech, and circulator. This is where instrument function (clamping, retracting, dissecting, suturing) directly matters.
  3. Post-operative phase — the patient moves to recovery, and used instruments return to sterile processing for decontamination, inspection, and reprocessing — closing the loop back to SPD.

Common Surgical Specialties and Their Instrument Needs

Different specialties rely on very different instrument sets, which is why instrument identification exams (like the CIS) organize content by specialty:

  • General Surgery — clamps, retractors, and scissors for abdominal and soft-tissue procedures
  • Orthopedics — rongeurs, osteotomes, rasps, and powered instruments for bone work
  • Neurosurgery — micro-instruments, specialized retractors (Leyla, Budde halo), and bipolar forceps for delicate tissue
  • ENT (Ear, Nose & Throat) — fine forceps, specialized punches, and endoscopic equipment
  • GYN — tenacula, dilators, and curettes for gynecological procedures
  • Cardiovascular — vascular clamps (Satinsky, DeBakey) designed to occlude vessels without crushing them
  • Ophthalmic — extremely delicate, small-scale instruments for eye procedures

How This Knowledge Helps You Pass Your Certification Exam

CRCST, CIS, and CER exam questions frequently frame instrument or process questions around a surgical scenario — "the OR is performing a craniotomy, which retractor is most likely needed?" or "a loaner tray arrives for a scheduled ortho case, what's your first step?" If you understand what the procedure actually involves, these scenario-based questions become much easier to reason through instead of relying on rote memorization.

Key Terms Every SPD Tech Should Know

  • Scrub tech — sterile team member who hands instruments directly to the surgeon
  • Circulator — non-sterile team member managing the OR from outside the sterile field
  • Case cart — the cart containing all instruments/supplies needed for a specific procedure
  • Loaner tray — instrument sets borrowed from a vendor for a specific case, requiring inspection before and after use

Understanding the surgical side of the OR — even without ever scrubbing in — makes instrument processing, tray assembly, and exam scenario questions click into place.
