Surgical technologists work directly in the sterile field, handing instruments and supplies to the surgeon in real time — which means anatomical knowledge isn't optional background information, it's a working requirement. This guide covers the core anatomy and physiology concepts every surgical tech candidate should know.

Why Anatomy Matters for Surgical Technologists

Unlike sterile processing technicians who primarily work behind the scenes, surgical technologists are in the OR during the procedure itself. Anticipating a surgeon's next instrument request, understanding patient positioning, and recognizing anatomical landmarks during a case all depend on a working knowledge of anatomy and physiology — not memorized trivia, but functional understanding you can apply live.

Anatomical Position and Body Planes

Every anatomical description starts from the standard anatomical position: standing upright, facing forward, arms at the sides, palms facing forward. From there, three body planes describe direction and location:

  • Sagittal plane — divides the body into left and right
  • Frontal (coronal) plane — divides the body into front (anterior) and back (posterior)
  • Transverse plane — divides the body into upper (superior) and lower (inferior)

Directional terms build on this: medial (toward the midline) vs. lateral (away from midline), proximal (closer to the point of attachment) vs. distal (farther away), and superficial vs. deep.

The Body Systems Most Relevant to Surgery

  • Integumentary system — skin, the first barrier breached in any surgical incision; understanding layers (epidermis, dermis, subcutaneous tissue) matters for incision and closure
  • Skeletal system — bone structure and joints, essential for orthopedic and neurosurgical cases
  • Muscular system — muscle groups and their role in retraction, exposure, and patient positioning
  • Cardiovascular system — major vessels are directly relevant to any procedure involving clamping, ligating, or repairing blood vessels
  • Nervous system — critical in neurosurgery and any procedure where nerve preservation is a surgical priority
  • Digestive and reproductive systems — relevant to general surgery and GYN procedures respectively

Patient Positioning and Anatomy

Common OR positions are chosen specifically based on the anatomy being accessed:

  • Supine — lying face-up; the most common general surgery position
  • Trendelenburg — supine with the body tilted head-down, used for pelvic and lower abdominal access
  • Lithotomy — on the back with legs raised and supported, used for GYN and some urological procedures
  • Lateral — lying on one side, common for thoracic and some orthopedic/renal procedures
  • Prone — lying face-down, used for spinal and some neurosurgical procedures

Understanding why a position is chosen (which anatomical structures it exposes) makes positioning knowledge far easier to retain than memorizing a list.

How This Applies to Exam Questions

Certification exams frequently test anatomy in an applied way rather than asking for pure definitions — for example, framing a question around a specific procedure and asking which position or approach would be used, or which anatomical landmark a specific instrument is designed to access. Building functional understanding (not just term memorization) pays off directly on test day.
