What Is a Biological Indicator (BI)?
A biological indicator contains a population of highly resistant, harmless bacterial spores that must be killed for a sterilization cycle to be considered successful. Unlike a CI, a BI directly tests whether microorganisms actually died — it's the closest thing to a direct test of sterility assurance.

When Must a Biological Indicator Be Run?

What Happens If a Biological Indicator Fails?
A positive BI result means the sterilization cycle failed to kill the test spores — indicating a real sterility assurance failure. This triggers an immediate recall protocol:
- Remove the sterilizer from service
- Recall all items processed since the last negative BI
- Notify the OR and any department where recalled items may already be in use
- Investigate the cause (loading error, mechanical failure, packaging issue)
- Re-test the sterilizer before returning it to service
